Orthodontics is the dentistry specialty dedicated to corrections of teeth-mandibular deformations and anomalies and the related functional problems. Orthodontic treatment should be held in two ways: with fixed technique (braces) or with removable apparatuses.


During an orthodontic treatment our main objective is to achieve the best functional and aesthetic result depending on the case. Thanks to the treatment the patient not only gets a beautiful and attractive smile, but it improves the patient's confidence and quality of life.


There are no age limitations for holding orthodontic treatment. Most of all it is important for the adolescents to be able to achieve very good results while guiding the mandibular growth.

First consulting is mainly about getting to know each other. After the primary examination of the person, bite and teeth, we discuss the need and possibilities of orthodontic treatment. We provide a short information about the forthcoming treatment and the need of dental treatment in view of the dentitions general condition.

In order to provide an exact analysis of the teeth-mandibular deformations and anomalies and accurate treatment plan, we should decide on the arrangement and condition of teeth, bite, location of mandibles to skull, periodontium (teeth-retaining apparatus), muscles, lips and tongue. For this we need the following:

Taking teeth prints and moulding orthodontic models.


Teeth and face photo shooting.
Before undertaking the orthodontic treatment we perform teeth and face photo shooting and analyse the profile, lips, smile line, facial symmetry.


Digital orthopantomography and teleroentgenography
During the orthopantomography we could see all teeth, teeth germs, general condition of dentition and bone, sinus level. During the teleroentgenography we decide on the ratio between the upper and lower mandible.

On the grounds of all analyses of orthodontic models, photo- and x-ray images, we elaborate an individual treatment plan according to the case. It includes individual appliances (braces, bows, orthodontic apparatuses etc.)

During the orthodontic treatment with fixed appliances (braces) or removable appliances the patient’s cooperation is essential (keeping up to the period for wearing the appliance, cares and maintenance of braces, hygiene of oral cavity and conformity with control examinations performed by the orthodontist).

During the orthodontic treatment of the teeth-mandibular problem has been successfully corrected and we have achieved a beautiful result. This result is to be maintained. Teeth need time to get fixed in the new position.


In order to achieve long-term success of the orthodontic treatment, we would like to recommend long-term retention. In order to maintain the result we use continuous lingual retention in the area of the frontal teeth. It is comfortable and cannot be noticed from the outside (while smiling). The patient is not supposed to wear it during the night (just like in the case of removable retainers) and there is no risk to forget about it.

“Perfect dentition” is not common. Teeth-mandibular deformations and anomalies are genetically predefined in a few cases. In most cases these have been caused by bad habits like thumb sucking, as well as losing deciduous tooth ahead of time that could hinder the eruption of permanent teeth.


The most common main reasons for orthodontic treatment are the following:

Bundled teeth
There is not enough space for teeth along the jaw. These are curved and rotated. In the case of great shortage of space teeth may not even erupt fully.


Tremas
Distances between teeth could result from the nonconformity between teeth size and mandible size (small teeth – large mandibles) or missing or extracted teeth.


Inclined and rotated teeth
The inclined and rotated teeth could result from the absence of space or extracted teeth. The frontal teeth that are greatly inclined frontwards could be caused by thumb sucking.


Teeth migration
In the case of adults the reason behind teeth relocation could be the loss of one or more teeth, as well as periodontitis.


Malocclusion
KL III bite – the lower mandible is located more frontwards than the upper mandible.
KL II bite – the lower mandible is located more backwards than the upper mandible.
Cross bite in the lateral section (unilaterally or bilaterally).


In view of a forthcoming treatment with crowns, bridges or implants, we could ensure good preparation with orthodontic treatment. Pre-prosthetic treatment is necessary to improve the masticatory function and aesthetics and ensure more functional and aesthetic prosthetic restoration.


Indications for pre-prosthetic orthodontic treatment:

Straightening inclined teeth;


Correction of migrated or rotated teeth;


Release of sufficient place for a bridge or implant;


Correction of bundled teeth in the frontal area;


Achieving a functional bite before the prosthetic treatment;


Removal of the preliminary contacts;


Improvement of incisal guidance and canine protection.

Orthodontic treatment could promote treatment and control of periodontium. The correction of the inaccurately arranged teeth (that existed before periodontitis or result from it), could significantly improve the situation and facilitate teeth clean-up that is paramount for keeping periodontium healthy.

The highly bundled teeth create very bad prerequisites for the teeth-retaining apparatus. The bone between the teeth roots is very thin and does not ensure good tooth retention. Dental plaque is harder to eliminate, it piles up at the borderline between gum and tooth and creates prerequisites for periodontitis development. Teeth levelling thanks to orthodontic treatment creates those niches for plaque retention and prerequisites for bone modelling.

This is an innovative system for teeth strengthening thanks to which patients could undertake their orthodontic treatment is an almost unnoticeable and more comfortable manner. Transparent and thin plastic braces (aligners) move teeth step by step. Braces are removable and the patient could take them down while eating and brushing teeth.

Treatment is to be performed with numerous very thin transparent braces that move teeth to their desirable position. Every brace set (night and day) are to be worn for 3 weeks. Then these are to be replaced with the next set and with every other brace teeth are moved to the distal position.


The patient feels very slight pressure onto teeth. Braces should be worn for around 22 hours in every 24-hour period except for while eating and washing teeth.


Braces OrthoCaps are produced in Germany. After taking an individual cast we prepare 3D models where we could simulate teeth movement and see the final result.


The treatment with OrthoCaps is applicable in the case of adults and teenagers with already shaped up permanent dentition. Nevertheless it is not applicable for all teeth-mandibular deformations and anomalies. We could decide on your suitability for treatment with OrthoCaps during the first consultation.

Casts for making the braces
Afterwards these casts are sent to the OrthoCaps laboratory in Germany and are being used for further treatment planning.


Online treatment plan
Onto 3D models we could simulate teeth movement and see the end result. From that 3D set up we could see the number of braces we would need for holding the treatment, as well as its duration. After the orthodontist and patient approve of the treatment plan, braces are to be elaborated.


Placement of the first braces OrthoCaps
Before placing the braces onto several teeth we put attachments (small buttons made of polymer with the tooth colour). After treatment these buttons are removed. They support teeth’s better relocation. After you are provided with clear information about treatment progression, you would be provided with another brace sets.


3-week period of brace replacement
Every treatment stage lasts 3 weeks. It is held with 2 types of braces – harder ones being worn throughout the day and softer ones – during the night. In every third week we replace the braces with the ones of the next stage.


Treatment control
Treatment is being monitored at regular control examinations at every 6-9 weeks. The treatment duration depends on the brace number.


Control casts
After the 8th brace we make control casts that are sent to Germany in order to follow up whether teeth are moving according to the plan. If we establish any lagging behind (if the patient didn’t wear the brace on a regular basis), we send the others, already corrected braces and discuss once again the period of wearing them (20-22 per 24-hour period).


Retention
After achieving the desired result, we place a certain type of retention as a rule.



Braces are most often used in permanent dentition and could correct inaccurately arranged teeth, as well as a faulty bite.


Braces are fixed with a special glue to the teeth. An elastic bow passes through them and it has memory. In the beginning it bends in order to ligate (bind) to the braces, then it tries resuming its initial shape that has certain impact onto the tooth thus slowly relocating them to the right position.


The clinic works with cutting-edge individual braces IP Appliance. Orthodontic analysis is performed via special orthodontic software that helps us choose the right appliances in view of the case (braces, rings and bows) bearing in mind the original shape of the dental bow, inclination of frontal teeth and the treatment objective. All this helps us implement treatment and achieve better results in shorter time.


Ceramic (white) braces are often preferred by patients because they resemble teeth colour and are less noticeable. This makes the treatment more discrete and patients – more confident.
